INSURANCE REFERRALS
PAA is committed to providing quality care to all our patients. Each insurance carrier has different coverage depending on the contract with that particular employer. It is important to be familiar with your insurance coverage!
Many of the managed care plans require referrals for specialists; therefore, we will need to have the referral before any services are rendered. In addition, you may need to obtain pre-certification from your insurance carrier before certain procedures and/or equipment may be ordered.
PAYMENT INFORMATION
Co-payments are collected at the time of the appointment. We accept Cash, Check, Visa, MasterCard, American Express and Discover.
Our staff will make every effort to collect outstanding balances after the time your follow up appointment. Any specific insurance coverage questions should be directed to our member services representative.
